Job Description
The Case Associate is responsible for supporting the team during outreach events, and assisting to monitor the well-being of clients assigned to him/her.
As part of the CREST team, you will be responsible to

support clients with or who are at risk of mental health issues and/or dementia by linking them to the necessary support and resources.

monitor and support clients in the community as assigned by the lead.

for organizing and conducting outreach to increase mental health and dementia awareness and promote early identification.

work with regional health services, Hospitals, volunteers, grassroots, schools, internal organisation departments etc. on activities and programmes for the seniors.
Essential Duties and Responsibilities:

Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.

Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.

Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.

Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.

Conduct regular visits to monitor the well-being of the client (NTUCH CCMS/CREST).

Support in initial assessment of walk-ins and referral cases, follow up with intervention plans for clients as per the prescribed care plan.

Involve in the evaluation and documentation of the programme.

Qualification
- Diploma in Social Service, Psychology or Counselling
