Job Description
Join us as a Case Executive and be part of our CREST-PDS team.
You will need to
- Provide case management support to clients who are newly diagnosed with dementia and their caregivers
- Provide care coordination and service linkage to clients with dementia and their caregivers, with the appropriate resources and support including caregiver support
- Participate in multi-disciplinary case discussions with hospital professionals relating to the well-being of clients who are diagnosed with dementia
- Conduct regular home visits to monitor the well-being of the caregiver and client with dementia
- Support the evaluation and documentation of the programme
- Support other ad-hoc assignments as assigned by CREST-PDS Lead/Department Head

You should
- Have good knowledge of the geriatrics and community services
- Be proficient in Google Suite applications
- Have strong communication and stakeholder management skills
- Be passionate about community services and be service-oriented
- Have good assessment and decision-making skills
- Be meticulous and enjoy working independently
Qualification
- Degree/Diploma in Social Work/Psychology/Counselling
