Job Description
We are looking for locum doctors to support the Seniors. Currently, you have the opportunity to work at any of our different settings - Nursing Homes, Family Medicine Clinics and Home Care services.
The Doctor is responsible for the clients’ care under NTUC Health, by working closely with a multidisciplinary team comprising care professionals like care support staff, nurses, and allied health professionals.
Professional Responsibilities
- Practice at all times in accordance with the SMC Physician's pledge and abide by the SMC Ethical Code of Conduct
- Keep abreast of medical information and developments through CME events
- Keep abreast of the group's medical practice
Responsibilities at Family Medicine Clinic
- Conduct reviews for chronic medical conditions and practice in a manner aligned with the patient’s goals of care, according to best medical evidence and practices.
- Diagnose and treat acute medical conditions, prescribe medications, perform minor surgical procedures and initiate referrals for specialist reviews where necessary
- Work with a team of fellow doctors, nurses and care coordinators in order to provide the best possible outcomes for our patients
- Support NTUC Health’s overarching strategic goals by engaging with community partners, grassroots organizations and regional health systems where opportunities arise
- Explore and take part in medical innovation and novel care delivery models
Responsibilities at Nursing Homes
- Conduct medical rounds to review nursing home residents on a regularly scheduled basis to attend to their chronic medical needs and acute conditions in a manner aligned with the patient’s goals of care, according to the best medical evidence and practices
- Write medical reports, memos, referrals and administrative forms where required for purposes of application to schemes beneficial to residents and/or to correspond with other healthcare and non-healthcare services
- Practice with a patient-centered approach, and engage with the resident or next of kin where necessary in order to develop a care plan aligned with the residents’ values and desired outcomes
- Lead a multi-disciplinary team of nurses, allied health professionals, social workers and welfare staff with regular multidisciplinary meetings in order to achieve patient-centred outcomes
- Guide care staff with regard to best medical practices
- Support NTUC Health’s overarching strategic goals by engaging with community partners, grassroots organizations and regional health systems where opportunities arise
- Explore and take part in medical innovation and novel care delivery models
Responsibilities at Home Care Services:
- Deliver patient-centred medical care in a manner consistent with the patient's individual care goals
- Conduct rounds at clients’ homes to ensure care plans are being carried out
- Conduct regular check-ups and diagnosis of ailments, prescribe medication and refer client(s) for further specialist check-ups where required
- Respond in a timely fashion to the client's change in function or condition
- Ensuring patients receive their test results in a timely fashion
- Ensuring proper management of patients' follow-ups and return visits
- Communicating with other doctors when referrals are necessary
- Be an active and contributing member of the Homecare team

Qualification
- Basic medical degree recognized by Singapore Medical Council
- Registered with Singapore Medical Council
- Relevant experience working with elderly patients, or those with Graduate Diploma in Geriatric Medicine / Family Medicine will be advantageous
- Possess good communication and interpersonal skills
- Willingness to serve and connect with the elderly and underprivileged
- Interest in medical innovation and novel care delivery models
