Job Description

Our goal is to provide suitable intervention care for seniors. This is done with a team of trained social workers using a case management approach, close monitoring and counselling services. As a Social Work Associate, you will be required to carry out casework and apply intervention strategies appropriately to improve the well-being of the client and support the families and caregivers in the community.

You will need to

  • Manage case referrals
  • Consult with the centre lead, conduct assessment, planning and intervention for cases
  • Monitor delivery of coordinated services
  • Adopt a suitable case management approach when working with clients
  • Refer and coordinate care services for clients to ensure they are adequately supported
  • Keep abreast and provide information on the various assistance schemes or care options available
  • Assess family financial status
  • Manage administration of grants, subsidies and support services where relevant or necessary
  • Ensure that all MOH service requirements on financial and quality aspects are adhered to

Qualification

You should:

  • For Social Worker/ Social Work Associate: Bachelor’s Degree preferably in Social Work
  • For Case Manager: Degree in counselling/Gerontology/psychology/sociology etc 
  • Minimum 2 years’ experience, preferably in community or healthcare settings in Singapore
  • Thrive in a collaborative and dynamic environment
  • Have strong communication and stakeholder management skills
  • Be proficient in Google Suite Applications

  • Have experience in Social Work 
  • Can converse in local languages to communicate better with our senior clients
