Job Description

Our goal is to provide suitable intervention care for seniors. This is done with a team of trained social workers using a case management approach, monitoring and counselling services. As a Social Worker, you will be required to carry out casework and apply intervention strategies appropriately to improve the well-being of the client and support the families and caregivers in the community.
You will need to:
- Responsible for managing case referrals to support clients’ needs
- Conduct assessment, planning and intervention for cases by engaging client and building rapport, performing needs assessment and advocating for clients’ needs
- Develop and review appropriate intervention strategies in consultation
- Refer and coordinate care services for clients
- Provide information on the various assistance schemes or care options available
- Raising awareness and facilitating advance care plan for seniors
- Conduct outreach activities to the community

Qualification
- Bachelor’s Degree preferably in Social Work
- Minimum 3 years’ experience, preferably in community or healthcare settings in Singapore
You should:
- Thrive in a collaborative and dynamic environment
- Have strong communication and stakeholder management skills
- Be proficient in Google Suite Applications
It would be great if you:
- Have experience in Social Work
- Are able to converse in local languages
