Job Description

Beside providing psychosocial support to seniors with end stage illnesses and their caregivers living in the community, this role requires the social worker to support the physical and mental well-being of seniors in the community by making assessment of their care needs and provide intervention to increase the coping capacity of seniors. The role focuses on helping seniors maintain dignity, comfort and quality of life while supporting families through end-of-life planning, caregiving, bereavement and coordination of care.

 

You will need to: 

Provide Basic Casework and Counselling:

  • In consultation with the program lead, conduct assessment, planning, and intervention for cases

    • Engage clients and build rapport 

    • Develop, implement and review individual care plans and intervention strategies

    • Perform needs assessment of clients and caregivers

    • Advocate for clients’ needs 

    • Coordinate services with other community resources on the client’s behalf 

    • Case documentation

  • Manage case referrals 

  • Conduct and participate in case conferences/MDT meetings 

  • Monitor delivery of coordinated services 

  • Document and evaluate case outcomes 

  • Adopt a case management approach when working with clients

    • i.e. collaborate with internal business units and other agencies/specialists to provide multi-disciplinary assessments and monitoring of clients

 

Share Information and Conduct Referrals

  • Refer and coordinate care services for clients to ensure they are adequately supported to continue living in the community

  • Provide information on assistance schemes or care options to help caregivers make informed decision about care and future planning

  • Support discussions on advance care planning and end-of-life preferences

 

Support Administrative Functions 

  • Maintain accurate records and update progress reports and BumbleBee timely

Additional Responsibilities: 

  • Raise awareness and facilitate Advance care plan for seniors

  • Conduct outreach activities to the community

  • Other ad-hoc assignments

Qualification

  • Graduate Diploma or Bachelor’s Degree in Social Work

  • Registered Social Worker with SASW

  • Minimum 2 years experience in community healthcare or palliative care settings in Singapore

  • Possess 1-2 years of experience in administration

Other Information

You should 

  • Have good knowledge of the palliative, geriatrics and     community services

  • Be proficient in MS Office applications

  • Good communication skills (oral and written)

  • Have strong assessment and problem solving skills 

  • Be detail oriented

  • Be comfortable working independently
