Job Description

As a Care Manager, you will be required to carry out basic casework and apply intervention strategies appropriately to improve the well-being of the client. 

Basic Casework and Counselling

  • Manage case referrals
  • In consultation with the ED, conduct assessment, planning and intervention for cases. Includes:
    • Engaging client and building rapport
    • Performing needs assessment
    • Advocating for clients’ needs
    • Coordinate services with other community resources on client’s behalf
    • Preparing case notes
    • Work with Caring Assistance from Neighbours level 1 (CAN1) carers to provide monitoring to clients
  • Conduct and participate in case conferences
  • Monitor delivery of coordinated services
  • Document and evaluate case outcomes
  • Develop and review appropriate intervention strategies in consultation with ED
  • Provide supervision, guidance and advice to support staff and volunteers (including and especially for CAN1 carers).
  • Adopt a case management approach when working with clients, i.e. network with other agencies/ specialist to provide multi-disciplinary assessments and monitoring of clients


Information & Referral

  • Refer and coordinate care services for clients to ensure they are adequately supported to continue living in the community.
  • Provide information on the various assistance schemes or care options available.


Administrative Functions

  • Maintain and update daily records


Job Requirements

  • For Care Manager: Bachelor’s Degree preferably in Social Work / Psychology / Sociology / Gerontology / Counselling
  • For Assistant Care Manager: Diploma with social service experience/ SSTI Diploma in Social Service/Gerontology
  • Minimum 3 years’ experience in community healthcare settings in Singapore
  • Good knowledge of the geriatrics and community services
  • Proficiency in MS Office applications
  • Good communication skills (oral and written)
  • Passion in community services and service oriented
  • Good judgement and decision making skills
  • Good in problem solving
  • Good record keeping abilities and attention for details
  • Possess 1-2 years of experience in administration 
  • Meticulous and independent
  • Ability to speak English, Mandarin and Malay will be an advantage
